I wish to become a member of PHOENIX SPORTS CLUB (1982) LIMITED which will entitle me and the members of my immediate family to use the facilities of the Club, and to participate in sport, subject to selection. When Children attain the age of 18 years, they must, at the next renewal date, become members in their own right.

Members are entitled to attend any social or general meeting called by the Club, to vote at such meetings, or by proxy vote where necessary. (One vote per membership)

Whilst still a member and up to one year after membership ceases, every member undertakes to contribute a maximum of £10.00 in the event of the Club being wound up.

I agree to abide by the rules, regulations and code of conduct of the Club at all times. I understand that if I agree to carry players in my vehicle, I do so having ascertained my insurance is adequate. I understand that the Club has the right to refuse or revoke membership at the Boards discretion. I state that myself / child is not registered with a team within the same league as I am joining. I have read and understand the Phoenix Sports Code of Conduct
